We also provide after-hours services through our After-Hours Nurse Advice Line to cater to those unpredictable childhood medical needs. Our After-Hours Nurse Advice Line is staffed by experienced Pediatric ER nurses who provide evidence-based information to ensure valid care advice.
We are currently accepting new patients and all major health insurances. We offer competitive self-pay rates for patients who do not have health insurance as well as membership plan. If you are transferring from another practice, please arrange to get medical and immunization records prior to your appointment. We can provide the necessary forms, so you are able to have a seamless transition to our office.

If you are transferring from another practice, please arrange to get medical and immunization records prior to your appointment. Also, please call to speak to our staff to help coordinate your needs. To save time during your first appointment, kindly review our office policies and privacy practices. Please print and sign the forms below and bring them prior to your scheduled appointments. Please arrive 15 mins prior to appointment to fill out any other necessary paperwork.

Prescriptions and Renewals
All prescriptions and refill authorizations should be requested as far in advance as possible. Please allow our office 1-2 business days to refill the requested medications. For ADHD medications please allow at least 72 hours in advance due to the possibility of shortages of these medications.
